A range of health checks are available for people at different times of their lives. Health checks create a valuable opportunity to identify health problems early, when they are easier to treat. Health checks also provide patients with dedicated time to meet with the Registered Nurse and GP and discuss their health and wellbeing goals.
Health assessments that are available at Health on Henley include:
-
- Aboriginal and Torres Strait Islander health assessment (also called the 715 Health Check).
-
- Heart health assessment for patients with or at risk of heart disease
-
- Type 2 diabetes risk evaluation, for people aged 40-49 years.
-
- Health assessment for people aged 45-49 years who are at risk of developing chronic disease
-
- Health assessment for people aged 75 years and older
-
- Health assessment for people with an intellectual disability
Each of these health assessments has specific eligibility criteria. Please speak to our team to identify which health assessments you are eligible for.
Please note: Health assessment appointments are bulk-billed at Health on Henley.
