If you’re a new patient, we require you to complete a ‘New Patient Information Form’. A digital version of the form will be sent to you via HotDoc. The form will also be available for you to complete on the day of your appointment so please allow 10-15 minutes to complete this.
If you would like to transfer your medical records to Health on Henley, please ask our reception team for a ‘Transfer of medical records’ form.
Please note, all new patient appointments are booked for 30 minutes. This enables us to get to know you and your health and medical history and provide you with the best quality care.
